Job roles and responsibilities are as follows:

  • Ensure the delivery of quality services to individuals

    • Quality services consist of providing companionship, encouraging and supporting the clients to become more independent. 

    • Treating them how you would treat one of your own, and more. 

    • Building strong relationships with them, by taking the time to get to know them. 

    • Reading the ISP/AMFM to better understand and care for them.

  • Ensure health and safety compliance and maintain quality of standards​

​​

  • Maintaining compliance by providing appropriate supervision. 

​​

  • Knowing, understanding and implementing clients outcomes and HPC supports in accordance with the HPC documentation paperwork, ISP, AMFM,

​​

  • Completing walkthroughs at the beginning and end of shifts to inform Leads of anything missing, broken, low on stock, etc. 

​​

  • Provide information, advice, and support to residents

    • Being sure to provide companionship and taking time to communicate with each client. 

    • Take the time to get to know the clients in your care. 

​​

  • Communicate with SSA’s, Caseworkers, Local Authorities and participate in meetings concerning clientele.

 

  • Maintain of client files

    • By completing ALL paperwork each shift including documenting on HPC paperwork and shift summaries. 

​​

  • Follow protocol regarding concerns via chain of command

    • Contacting the assigned “House Leads” before contacting “House Manager” or CEO. 

    • When calling off make sure to contact the appropriate personnel.  

​​

  • Work varied hours

    • Work scheduled shifts, showing up on time. 

​​

  • Always maintain professionalism

​​

  • Conduct all other duties assigned by management

​​

  • Will travel to multiple locations to manage client concerns

    • When traveling be sure to complete NMT transportation documentation whenever transportation is provided. 

​​

  • Participate in case reviews

​​

  • Maintain day to day operations of home during shifts

    • Including cleaning through out the shift, completing documentation, properly supervising, not playing on phone/video games/tablets/etc when DSP should be interacting with client, being supportive and encouraging. 

​​

  • Report to house lead

​​

  • Assist clients with daily living tasks, personal hygiene/care, meal preparation (cooking, cleaning, laundry, and running errands), household chores, medication administration, encouraging them to maintain proper hygiene.

 

  • Implement client schedules and manage appointments

    • Communicate when appointments are made and approaching 

​​

  • Transport residents to outing (shopping and/or other activities)

    • Complete transportation documentation

​​

  • Provide companionship to the clients

​​

  • Report any changes to house lead

​​

  • Instruct and advise clients on household cleanliness, hygiene, nutrition, and finances

    • LEAD BY EXAMPLE! Be sure to assist with cleaning. We must ALL DO OUR PART!

​​

  • Evaluate client needs and communicate plan for continued services

​​

  • Document and initial that you have read logs

    • Make sure to print, initial and sign the back of the documentation paperwork.

 

  • Communicate all shift swapping/changes to scheduling personnel

​​

  • Turn in all receipts from outings to house lead. 

    • Store receipts properly until you are able to submit them

​​

  • Utilize company vehicle for client transportation only with prior authorization from house manager

​​

  • Ensure proper completion of all documentation and submission in timely manger

​​

  • Attend all mandatory trainings

​​

  • Report MUI’S and UI’s immediately to house lead
